Crown House Surgery is seeking experienced and motivated Advanced Clinical Practitioners. This role is ARRS funded and offers the opportunity to work in a supportive, modern environment where your clinical skills and professional development are valued.
About the Role
The successful candidate will:
* Work autonomously within your professional scope of practice.
* Manage minor illnesses and chronic disease exacerbations (e.g. COPD, heart failure).
* Provide same‑day patient assessments for acute and semi‑acute conditions.
* Conduct up to two home visits per day as required.
* Contribute to the smooth daily running of the practice, supporting our commitment to high‑quality patient care.
About You
You will be:
* A qualified and registered ACP with a relevant professional body.
* Experienced in primary care or similar settings.
* Able to work independently and as part of a multidisciplinary team.
* Enthusiastic, organised, and patient‑focused.

* An experienced nursing team with chronic disease and treatment room expertise.
* Pharmacists, Pharmacy Technicians, and a Social Prescriber.

Main Responsibilities
Key duties include:
* Registered with HCPC or NMC with Advanced Clinical Practice.
* Independent Prescriber.
* Previous experience working in general practice/primary care.
* Undertake full clinical assessment and prescribe autonomously.
* Able to manage acute and chronic conditions, conduct assessments, and make referrals as appropriate.
* Manage undifferentiated undiagnosed conditions, identify red flags and underlying serious pathology, and take appropriate action.
* Makes critical clinical judgements to satisfy expectations and demands of the job (e.g., first contact with patients presenting acutely) and manages care in the presence and absence of protocols.
* Make decisions where appropriate without recourse to others (e.g., direct referral of patients to hospital consultants without prior reference to other medical colleagues).
* Use complex decision making to inform diagnosis, investigation, and complete management of episodes of care within a broad scope of practice.
* Actively take a personalised care approach and population‑centred care approach to enable shared decision‑making with the presenting person.
* Excellent communication and teamwork skills.
* Committed to delivering high standards of patient care.
Qualifications
* Registered Nurse (Adult) or Advanced Physical Assessment & Consultation Skills (APACS) & minor illness modules.
* Master's degree or equivalent in Advanced Clinical Practice with digital badge.
* Non‑medical prescribing qualification.
* Good clinical IT system knowledge.
Experience
* Has previously worked in a primary care setting or a Primary Care Network.
* Ability to listen, empathise with people and provide person‑centred support in a non‑judgemental way.
* Courteous, respectful and helpful at all times.
* Able to get along with people from all backgrounds and communities, respecting lifestyles and diversity.
* Commitment to reducing health inequalities and proactively working to reach people from all communities.
* Ability to work on own initiative.
* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face.
* Ability to identify risk and assess/manage risk when working with individuals.
* Full UK driving licence & access to a car for home visits.
