Job overview
We are seeking an innovative, dynamic practitioner to provide person‑centered care for older adults experiencing mental health problems. The role is based at Royal Cornwall Hospital, Truro, with outreach to local community hospitals and community visits.
Main duties of the job
- Specialise in older adult psychiatric liaison care.
- Conduct psychosocial assessments, risk assessments and holistic discharge planning.
- Lead and supervise staff, and contribute to quality improvement, audit, teaching and service development.

Person specification
Education
- Current professional registration – NMC, HCPC.
- Evidence of continued professional development.
- Mentoring qualification.
Skills and attitude
- Experience working with people with dementia post qualification.
- Previous teaching/training experience.
- Experience in complex case management.
Knowledge and ability
- Ability to present informal and formal training to a range of parties including carers.
- Proven leadership, organisational, prioritising and team‑working skills.
