Role Overview
Professional positions advertised have arisen as a result of investment in and development of our service in Planned Care. The post involves supporting adults in their own homes to live as independently as possible through meaningful activity. The service is fast paced and supports customers long term. Our customers are often frail and frequently have more than one health condition, including varying degrees of physical and cognitive impairment. The care team works closely with our well‑established Multi‑Disciplinary Team members including Social Workers, Therapists and Community Nurses.
Key Responsibilities
* Support adults in their own homes to live as independently as possible through meaningful activity.
* Provide and promote person‑centred care and support to vulnerable customers who require differing levels of support to meet a range of complex social and health needs.
* Support a team to enable people to achieve their maximum level of independence through appropriate physical, emotional and psychological support including intimate personal care, support with basic health needs, practical tasks, appropriate communication and community involvement.
* Follow therapy plans and use innovative technology (training provided).
These principal responsibilities are not an exhaustive list of duties, only a general guide to the role. In consultation with the post holder, the duties may change from time to time to reflect the changing needs of the customer/service.
Requirements
All applicants must have a full UK driving licence and the use of a vehicle for work purposes.
Experience in providing personal care and supporting adults with complex health needs.
Ability to follow therapy plans and utilize innovative technology.
