This temporary position spans 24 weeks and offers a competitive pay rate of £38.82 per hour for a full 37-hour workweek. What you will do:
* Manage a caseload of individuals within secondary mental health services, ensuring each has a comprehensive Care and Treatment Plan (CTP).
* Apply relevant legislative frameworks, including the Mental Health Act and the Social Services and Wellbeing (Wales) Act, to deliver lawful, proportionate, and person‑centred interventions.
* Collaborate closely with colleagues, service users, and partner agencies to build effective working relationships and ensure coordinated care and support.
* Contribute to the team’s duty rota by responding to general enquiries and providing support to the team and service users as required.
* Demonstrate a person‑centred approach, ensuring high‑quality practice and positive outcomes for service users while maintaining strong communication and organisational skills.
