Overview
We have an opportunity to work as part of a small team providing a specialist service to people living with dementia in a community setting within Telford and Wrekin. Our service aims to provide support and advice to patients and their families on how to deal with communication and swallowing difficulties related to dementia. Approximately 25% of the role is dedicated to education of patients, families and staff. There is also the support of an assistant practitioner. The successful candidate will work as part of a small specialist team based in Telford providing a service to people living with dementia, their families and carers.
Responsibilities
* Provide assessment and management for a wide range of communication and swallowing disorders related to dementia.
* Deliver training to families and staff in a number of different settings (approximately 25% of the role).
* Work within the dementia team as part of the wider community SLT service covering Shropshire and Telford & Wrekin.
* Contribute to service delivery across clinical areas including community hospitals (ward-based and outpatient), in-reach to IDT and domiciliary teams, and involvement with specialist units as applicable.
* Support joint ENT clinics and SLT-led nasendoscopy clinics; participate in video fluoroscopy clinics and development of FEES services.
* Engage in regular clinical supervision, management supervision, and CPD activities to support colleague wellbeing and professional development.
Qualifications & Experience
* Qualified Speech and Language Therapist with relevant experience in dementia and related communication and swallowing disorders.
* Ability to provide education and training to patients, families and staff across settings.
* Experience working in community SLT teams and with multidisciplinary colleagues.
* Commitment to supervision, CPD, and professional development.
