We are offering an exciting opportunity for an Advanced Clinical Practitioner to join North Mersey Crisis Resolution and Home Treatment Team. This role will require expert assessment, planning, implementation and evaluation of care for people requiring urgent support with their mental health. This post will be based at Broadoak Unit, but practitioners will cover a footprint across Liverpool and Sefton. There may also be expectation for practitioners to support across areas of Mid Mersey depending on service need.
Responsibilities
* Provide strategic clinical leadership in relation to an area of specialism using the most up to date and evidence based theory.
* Undertake expert assessment, planning, implementation and evaluation of care for a specific group of service users.
* Offer expert clinical advice for service users, carers, staff and other professionals.
* Demonstrate advanced competence in highly complex decision making, assessment and the management of clinical needs within the specialist area of practice.
* Support and complement the existing medical arrangements.
* Work in partnership with service users, carers and other agencies to ensure effectiveness of outcomes; work as advanced clinical lead for an area of specialist practice across a range of services within the Urgent Care.
* Utilise expert mental and physical health professional skills to provide assessment, diagnosis, intervention and clinical guidance as appropriate, whilst responding to changes to the service user physical and mental health.
* Formulate, implement and evaluate interventions with individuals and their carers where appropriate.
* Make and receive referrals as an advanced autonomous practitioner.
* Demonstrate advanced knowledge and skills within a defined area of expertise.
* Critically appraise, evaluate and apply knowledge of theoretical frameworks and research evidence within a specialist area of practice.
* Visit service users in the most appropriate setting to meet their needs as required.
* Refer service users to other members of the multi-disciplinary team or specialism as their needs require.
* Use independent non-medical prescribing skills in the initiation and evaluation of treatment plans.
* Establish consistent and evidence based side-effect medication monitoring, acting and findings as necessary.
* Lead on the transition from medical led services to practitioner services within a specialist area of practice.
* Discharge from a specialist area of practice with an effective discharge plan of care in place.
* Liaise with other services as required.
* Undertake physical examinations of service users as required.
* Refer for medical intervention as the service users needs require.
* Adapt clinical practices of the specialist team based on evidence, policy and identified needs of service users.
* Ensure that clinical and non-clinical risk is identified and dealt with appropriately; report to the Local Division management team any risk that cannot be managed at an acceptable level without external support.
* Provide verbal or written reports as requested by other departments or the Local Division management team as required.
* Identify areas for service development in your area of responsibility.
* Adhere to Trust policies and report any failures in adherence to Trust Policy; contribute to development of Trust Policies and Local Division Guidelines.
* Maintain high standards in clinical expertise; monitor and present data relating to the role; participate in service meetings; offer and participate in regular supervision.
* Establish priorities and organise workload to ensure effective use of time; ensure accurate recordings of all referrals, caseload and outcome data; maintain systematic records of treatment and progress with written reports as required; provide statistical returns; produce full evaluation of service and its impact on an annual basis.
Organisational and Professional/Quality Expectations
* Adhere to the Trust's rights based approach, understanding service user choice.
* Take reasonable care for health and safety of yourself and others; participate in assessments and action plans to address safety issues.
* Ensure familiarity with post terms and conditions; document all incidents or near misses per Trust Policy and take action to minimise risk of repeat.
* Maintain acceptable standards of practice and confidentiality; participate in staff appraisals; conduct oneself with professional conduct and care in accordance with relevant bodies.
* Comply with the Mental Health Act (1983) and work within the Code of Practice Guidelines; stay updated with guidelines, protocols and processes relating to clinical care.
* Attend relevant meetings, courses and seminars; critically analyse information informing non-medical prescribing decisions; prescribe safely and cost effectively; apply knowledge of drug actions in prescribing; work within the prescribing legislative framework; keep up to date with mental health developments to ensure evidence-based prescribing.
* Maintain own professional qualifications and participate as a supervisor and supervisee within the Trust's Performance Management/Supervision framework; work towards goals and objectives as agreed in PACE; undertake mandatory training as stipulated by the Trust.
* Work closely with the Consultant Psychiatrist for their specialist area and other lead professionals within the Division.
