12.5hours per week; usual working pattern 1 x 12.5hr full day or night shift on arota basis, 07:30-20:00, and 19:30-08:00

Main duties of the job
We are a forwardthinking, inclusive registered Charity with Inpatient, Outpatient and Communityservices, providing care and support to palliative patients and their familiesin their own homes and at St Teresas Hospice.
In this post, you willprovide high quality holistic nursing care to palliative patients within theInpatient Unit and create appropriate plans of care to enable and recognizeindividual wishes and preferences. You will develop and demonstrate a sound knowledgeof palliative care and contribute to the future development of this nurse-ledunit through further expansion of specialist knowledge and skills as required.
Essentialqualifications and skills include:
* Firstlevel Registered Nurse (General/Adult Nursing) with current NMC registration
* Understandingof the Hospice setting and ethos
* Professionalexperience in the field of palliative care would be an advantage

Job responsibilities
To provide high quality holistic nursing care topalliative patients within the in-patient unit and create appropriate plans ofcare to enable and recognize individual wishes and preferences. To develop and demonstratea sound knowledge of palliative care and contribute to the future developmentof this nurse-led unit through further expansion of specialist knowledge andskills as required. To be willing to demonstrate own skills to new or juniormembers of staff and provide mentorship and act as a role model at all times.To be active in the promotion of effective communication between alldisciplines involved in the welfare of the patients both within the Hospice andwith external agencies.

Person Specification
Experience
* Clear understanding of and experience of multidisciplinary team working
* Post qualification experience working at NHS Agenda for Change band 5 (or equivalent) or above (desirable for IPU candidates, essential for Community candidates)
* Professional experience in the field of palliative care in any setting, including as a student
* Experience of community nursing
* Experience of public engagement
* Experience of using SystmOne patient record database
Qualifications
* First level Registered Nurse (General/Adult Nursing) with current NMC registration
* Understanding of the Hospice setting and ethos
* Evidence of teaching or mentorship in practice
* Palliative care qualification, i.e. An introduction to patients with cancer and long term conditions at Diploma level or BSc (hons) Practice Innovation (Palliative and End of Life Care)
* Non-medical Prescribing Qualification
* Willingness to undertake Nurse Prescribing qualification and Palliative Care Qualification if not already held
* Knowledge and understanding of the palliative care agenda including local and national guidelines and documents.
* Knowledge and/or experience of audit
