Care Coordinator

Reports To: Care Manager / Director
Position Summary
The Care Coordinator supports Service Users by organizing and coordinating services that promote positive health and wellness outcomes. This role works collaboratively with Service Users, families, providers, and community partners to ensure continuity of care. The position includes participation in an on-call rotation to respond to urgent needs and emergencies.
Key Responsibilities
* Conduct initial and ongoing Service User assessments, including medical, behavioral, and social needs.
* Develop and monitor individualized care plans in collaboration with interdisciplinary teams.
* Coordinate services and referrals across healthcare, behavioral health, and community-based providers.
* Educate Service Users and families on care plans, available resources, and self-management strategies.
* Monitor progress and adjust care plans as needed to support goal achievement.
* Document care coordination activities according to organizational standards.
* Advocate for Service Users to ensure access to appropriate services and supports.
* Participate in team meetings, case reviews, and discharge planning as required.
On-Call & Emergency Responsibilities
* Participate in scheduled on-call rotation, including evenings, weekends, and holidays.
* Respond promptly to urgent Service User concerns or crisis situations.
* Conduct risk assessments and provide crisis intervention support when necessary.
* Coordinate emergency services and communicate critical incidents to leadership.
* Ensure follow-up and continuity of care after emergency situations.
Experience
* Minimum of 2 years' experience in healthcare, case management, or social services preferred.
* Crisis intervention experience strongly preferred.
* Experience working with diverse populations and community resources preferred.

Job Type: Part-time
Pay: From £15,609.86 per year
Expected hours: 20 per week
