What the Job Involves As a Leaving Care Worker, you serve as a pivotal contact for young people aged 16 to 25, ensuring they transition safely and positively into adulthood. The role requires direct interaction with young people, as well as coordination with their families and external agencies, to execute personalized support plans. You'll engage with young people who may be at risk of poor outcomes, providing targeted interventions and coordinating resources. Additionally, you will contribute to a duty service supporting young people in crisis and will be responsible for maintaining accurate records and reports, participating in case conferences, and adhering to all relevant legislative and policy frameworks.
