As our Lead Reablement Worker, you'll be at the heart of supporting our customers aged 55+ to stay independent in their own homes. You'll lead our team of Reablement Workers, helping them deliver person-centred care that makes a real difference. Your role includes supporting daily living, coordinating care plans, and working with health professionals to help customers live well and stay connected to their community.
Typical day as a Lead Reablement Worker
* Supporting customers with needs like dementia and frailty to regain or maintain independence.
* Developing and reviewing personalised care plans focusing on reablement, safety, and customer aspirations.
* Providing guidance and leadership to Reablement Workers to ensure high-quality, person-centred care.
* Managing complex personal care needs, including stoma and catheter care, while promoting customer dignity and choice.
* Collaborating with external health and social care professionals for optimal support.
* Using assistive technology and creative solutions to empower customers to live independently.

You bring
* Experience in a similar role with understanding of reablement and independence empowerment.
* Adult Care Level 3 diploma (or Level 2 with relevant experience and willingness to study for Level 3).
* Strong leadership skills, with experience mentoring or guiding teams.
* Knowledge of CQC standards, risk management, and safeguarding.
* Ability to build trusting relationships with customers, families, and professionals.
* A passion for helping people live their best lives.

Job Details
* Working on a rota basis, with shifts between 7am and 10pm, including days, evenings, weekends, and bank holidays.
* Occasional night shifts during service setup or emergencies, flexibility required.
* Enhanced DBS check required, paid for by us.
* Comfortable using technology to manage care plans and records.
