This role is an integral part of the multi-disciplinary team at Riverside Health Partnership, driving positive changes in healthcare for local people. We are looking for a professional with a prescribing qualification and proven management experience, with skills to lead a team.
Main duties of the job
To work as part of the management team in the practice and support achieving efficient and effective prescribing activities. To promote high-quality, cost-effective, and evidence-based use of medicines within primary care, working alongside the Nottinghamshire Integrated Care Partnership. To provide leadership to the pharmacy and medicines management team, including mentoring, support, and professional development. Working as part of a multi-disciplinary team in a patient-facing role, to provide primary support to general practice staff regarding prescription and medication queries. Ensuring the practice integrates with community and hospital pharmacy to utilize skill mix and improve patient outcomes.

Job responsibilities
Main Duties:
1. Provide leadership to the pharmacy and medicines management team, including mentoring, support, and professional development.
2. Develop effective protocols for the team to ensure efficient and effective processes for patients.
3. Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
4. Take responsibility for the care management of patients with chronic diseases and undertake structured medication reviews to proactively manage people with complex polypharmacy.
5. Ensure patients have appropriate monitoring tests in place when required.
6. Provide specialist expertise in the use of medicines, addressing public health and social care needs, and tackling inequalities.
7. Provide leadership on person-centred medicines optimisation, including conserving antibiotics in line with antimicrobial stewardship guidance, and contribute to quality improvement initiatives.
8. Support patients to take their medications effectively, reduce waste, and promote self-care through structured medication reviews.
9. Support the integration of general practice with wider healthcare teams to improve patient outcomes and access to healthcare.
10. Assist in identifying cost-effective, evidence-based medicine use.
11. Reconcile hospital discharge letters, ensuring data accuracy on the clinical system.
12. Participate in prescribing audits, provide feedback, and implement changes with the practice team.
13. Answer medicine-related enquiries from GPs, staff, and patients, suggesting solutions and monitoring effects.
14. Analyze and interpret medicines data to support decision-making.
15. Contribute to developing and implementing new services with medicinal components.
16. Reduce medication errors by implementing changes following alerts and guidance.
17. Ensure practice compliance with CQC standards regarding medicines, including audits.
18. Contribute to PCN quality standards and maintain patient confidentiality in records.
19. Stay updated with current pharmacy practices and attend meetings as required.
20. Represent the practice at locality Prescribing Leads Meetings and develop relationships with pharmacy professionals across the system.
21. Respond to patient complaints per the practice procedures.
22. Abide by the General Pharmaceutical Society's code of ethics, ensuring all professional, ethical, and legal obligations are observed.
Management Duties:
1. Provide management and leadership to the pharmacy team, including mentoring, support, and professional development.
2. Promote a safe, effective working environment providing high-quality patient services.
3. Ensure adequate staffing cover and proactively develop the team to meet strategic aims.
4. Conduct regular audits and manage own time effectively.
5. Participate in staff appraisals and team meetings, assist in recruitment, and ensure ongoing training for team members.
Person Specification
Qualifications
* Masters degree in pharmacy or equivalent
* Postgraduate clinical qualification or relevant clinical experience
* Registered with the General Pharmaceutical Council
* Qualified prescriber
* Minimum of 2 years experience as a pharmacist in relevant settings
* Experience managing a team
* Experience working with the public and responsible for professional actions
* Experience developing pharmacy services
* Knowledge of PCNs, General Practice, and evidence-based medicine
* Ability to analyze medicines data and communicate complex information effectively
* Completed CPPE Clinical Pharmacist 18-month pathway or equivalent primary care experience
