Job Title: Advanced Practitioner - Hospital Team Location: Barnsley Pay Rate: £37 per hour Job Description: Liquid Personnel is seeking a dedicated and experienced Adult Advanced Practitioner to join our client’s Hospital Team in Warrington. What will your responsibilities be? In this role, you will: Lead complex discharge planning and assessments, ensuring safe transitions of care from hospital to community services, with a focus on timely interventions and reducing delayed discharges. Provide expert guidance and decision‑making support to social workers and multi‑disciplinary colleagues, particularly around safeguarding, Mental Capacity Act, and Continuing Healthcare considerations. Coordinate with clients, families, and professionals in a fast‑paced hospital environment, balancing clinical priorities with social care responsibilities to deliver integrated, person‑centred outcomes.
