Overview

Role Background

Our Personal Health Budgets service helps individuals transition from hospital back into the community by facilitating access to goods and services that promote recovery, independence, and overall well-being. Our SMI-LE service provides tailored support to people in the community who are experiencing mental health challenges. This includes assistance in key areas such as mental health and emotional well-being; loneliness and social isolation; social housing; welfare, benefits, debt management and employment.

You will be joining a supportive and growing team across our Adults and Older Adults portfolio, all working towards the same aim of ensuring that no one has to face a mental health problem alone. We’re looking for someone who is passionate about making a difference, who is organised and able to manage competing priorities, and who is able to work autonomously as well as contributing to our wider teams.

Purpose of the role

As a Wellbeing Practitioner, you’ll provide one-to-one support to adults facing moderate to severe mental health challenges. You’ll help individuals improve their wellbeing by addressing issues such as social isolation, housing, benefits, employment, and financial difficulties. You’ll design person-centred plans of care, and support people to settle back into the community following a hospital stay.

A key element of this role involves working closely with secondary care colleagues such as care coordinators. There is also an expectation that you will be based on site in Community Mental Health Team bases.

Support is personalised to each individual and can be provided in hospitals, at home, within the community, or through remote services. You’ll also signpost and refer service members to other local services for continued support. Travel across Bradford and Craven is required, and you'll work closely with the wider team to ensure coordinated, person-centred care.

Role Outcomes

* Complete 1- to 1 support for adults facing moderate to severe mental health challenges face-to-face and/or over the telephone
* Provide support that meets the service member needs around client wellbeing, such as employment, welfare and benefits, social housing, money management, social isolation, and bereavement
* Have a good understanding of support services across the district and signpost/complete onward referrals for service members to relevant services for additional support
* Work in partnership with other services across Bradford District and Craven to ensure the best outcome for service members
* Record information electronically and maintain up to date service member records
* Take responsibility for ensuring Safeguarding, Information Governance and Equality & Diversity requirements, and all other Company policies are complied with and seek to improve these where possible
* Work in alignment with the policies, aims, objectives and values of Bradford District and Craven Mind
* Perform any other duties from time to time that may be reasonably required to ensure the smooth running of Mind in Bradford
* Work closely with finance department to process purchase orders and understand BDC Mind financial procedures

Qualifications

* Level 3 or above qualification in a relevant field or at least 2 years relevant experience

Experience

* Understanding of the key issues within mental health, and the potential challenges that people may face
* Knowledge of other relevant health & social care organisations across the Bradford District & Craven
* Knowledge of risk management and health and safety issues
* Understanding of safeguarding and good practice
* Understanding of confidentiality and data protection
* Experience of barriers people face following discharge from a health setting
* Experience of housing application and barriers for re housing Benefit application and processes

Skills and abilities

* Ability to gather relevant information to support and assess risks
* Excellent proven communication and organisational skills, including an ability to encourage and engage people in ways to make positive changes in their lives
* Excellent IT skills, including use of Microsoft Packages
* Ability to work as part of a team and work alone and use own initiative
