We are currently seeking a capable, enthusiastic, and committed Care Co-ordinator to join our hardworking and friendly administrative team. We require an individual who is keen to work in a person-centred way with patients and colleagues to encourage patient engagement in the quality improvement programmes, appropriately supporting those who may experience additional barriers to accessing the services.
This role is dynamic, multifaceted, and always evolving to provide streamlined administrative support for the practice team in delivering the outcomes from the Network Direct Enhanced Service (DES) Agreements. Prior experience with SystmOne and the NHS is advantageous but not mandatory.
Main duties of the job
KEY RESPONSIBILITIES
The role requires strong administrative skills and the ability to quickly learn a good working knowledge of SystmOne, including running searches to identify patient cohorts as directed by clinicians.
Responsibilities include clinical audit and evaluation related to Network DES clinical outcomes and quality improvement initiatives. Managing digital health app dashboards, inviting patients for NHS health checks, learning disability reviews, and mental health reviews.
Supporting patients with mental health issues using agreed pathways for triage to clinicians and local services. Supporting hypertension pathways, triage of blood pressure results, onboarding patients to platforms, and liaising with the Digital Transformation Lead to meet CCLIP targets.
Checking Shared Care requirements for secondary care patients, booking follow-ups or phlebotomy, monitoring Diabetes 8 Care Processes, importing data, and flagging patients needing additional support.

Job responsibilities
The role requires a good working knowledge of SystmOne and understanding of the Dorset Intelligence & Insight Service (DiiS); training will be provided if needed.
Undertaking non-clinical Cancer Care Reviews via phone, monitoring diagnostic testing requirements, making Pharmrefer referrals, maintaining Spirometry waiting lists, and managing digital health dashboards.
Inviting patients for health checks, dementia reviews, learning disability reviews, and mental health reviews. Supporting the achievement of QOF and IIF targets through accurate clinical coding.
Maintaining records of NHS Digital Flags and communication adjustments for patients with disabilities, supporting Safeguarding MDTs, and monitoring diabetes care processes.
Booking structured medication reviews, supporting mental health referrals, creating referral letters, and using clinical letters to update patient records.
The role also involves working in a person-centred way to encourage patient engagement, supporting quality improvement initiatives, and contributing to a positive service-user experience.
Additional duties include supporting clinical audits, evaluations, and undertaking any other duties as required.
