Hays Healthcare are actively recruiting Band 6 Mental Health Practitioner (RMN) withmunity and or A&E experience in Stockport.
Your new role and responsibilities
The Mental Health Practitioner service offers aprehensive range of mental health specialities within one multidisciplinary team so that all patients with a suspected or actual mental health problem can be assessed, treated, signposted, or referred appropriately regardless of age, presentingplaint, time of presentation or severity.
1. You will undertake telephone triages, urgent and routine face to face assessments and onward referral to the most appropriate service, getting people the right care, at the right time and by the right service.
2. You will undertakeprehensive assessments of the health and social care needs of individuals who are referred to the service, in addition toprehensive risk assessments
3. To participate in the duty worker rotation
4. To participate in the triage process of new referrals to the service.
5. To develop individualised packages of care for service users with mental health problems, including signposting where necessary.
What you'll need to succeed
6. Co-ordinate and prioritise the day-to-day clinical workload relating to referrals received by the liaison team.
7. Undertake timely mental health assessments with service users of all ages presenting with mental health problems to the accident and emergency department and within the general hospital.
8. Be a source of specialist knowledge and advice to professionals at all levels within the general hospital around care and management of service users presenting with primary and secondary mental health needs.
9. Be responsible for the development of care plans with service users and other relevant stakeholders, if appropriate this will include the autonomous discharge. As part of this role, it is expected that the post holder will liaise andmunicate with appropriate statutory and non-statutory agencies or care providers when developing care plans.
10. Must have A&E experience
11. Assess individuals with a range ofplex needs, such as organic and functional illness, this would include dementia and a range of illnesses associated with people from later life. In addition, there will be a requirement to assess individuals with learning disabilities. As well as CAMHS for the younger service users we see.
