We are actively searching for qualified Health Visitors and School Nurses who are eager to enhance their leadership capabilities and contribute to the future of Public Health Nursing in Croydon. We seek individuals enthusiastic about collaborative work, possessing warmth, excellent communication skills, and a solid understanding of the Healthy Child Programme and Safeguarding policies. Our dynamic learning and development team offers an exciting journey within our service.

This role is a key part of a multidisciplinary team dedicated to delivering the Healthy Child Programme and supporting families with children aged 0-5. The aim is to implement an integrated approach to Health Visiting across Croydon Health Services. The post holder will lead and deliver the Healthy Child Programme by:

1. Managing a designated caseload: Working as the named health professional responsible for planning and delivering the HCP, reporting to and being line managed by a Team Leader or Clinical Service Lead (CSL).
2. Workload planning: Effectively planning their own workload to meet client needs, clinical activity targets, and KPIs.
3. Partnership working: Collaborating with team members, infant feeding colleagues, service users, Croydon Health Service (CHS) staff, and stakeholders to promote the health and wellbeing of children, young people (age 0-19), and their families.
4. Clinical practice development: Working with the CSL, Practice Educator, and multidisciplinary teams to develop innovative clinical practices aligned with the Healthy Child Programme.
5. Assessing needs: Using tools like Family Strengths and Needs, Early Help Assessments, and Risk Assessments to plan, implement, and evaluate evidence-based care, especially for families with chronic illnesses or disabilities, and advising on care in cooperation with other agencies.
6. Effective liaison: Coordinating with colleagues within Croydon Health Services and partner organizations to ensure timely and well-coordinated care for children and families.
