Croydon Outcome Based Commissioning Alliance has developed six Integrated Care Networks (ICNs) across the borough and the Nursing Associate will work as part of the Complex Care Team with Croydon Health Services.
The Nursing Associate will deliver high quality, personalised, compassionate care to service users. They will carry out tasks autonomously with the support of a Registered Nurse.
Responsibilities
* Autonomously implement quality nursing care, including physical, psychological, social, recreational and spiritual support.
* Ensure fundamentals of care: nutrition, hydration, hygiene, comfort, emotional and social support.
* Conduct initial data collection, monitor progress, provide feedback, produce reports and in‑care reviews, and devise plan of care under supervision.
* Administer routine medications following successful completion of competency training.
* Escort service users and facilitate admission and discharge processes, including property checks, orientation and community support.
* Advocate for service users’ needs and rights, empowering them to take responsibility for their care within Trust policy.
* Ensure safety and security, report concerns, and maintain confidential records in line with Trust policy.
* Co‑ordinate and prioritise activities to support the day‑to‑day running of the clinical area.
* Support junior staff, provide 1‑to‑1 engagement, and contribute to the development of care plans in partnership with the multidisciplinary team.
* Participate in reflective practice, supervision, CPD training and quality improvement initiatives.
