What the Job Involves This role involves assessing citizens in line with statutory duties, planning and commissioning services to overcome barriers for adults with care and support needs, and monitoring care plans. Working in partnership with citizens, carers, and adults at risk, you will coordinate resources, maintain accurate case files, and ensure compliance with council policies. You will also supervise or mentor staff, take part in multidisciplinary team activities, and participate in personal development and training.
