Do you have a specific interest in caring for patients requiring structural heart procedures such as TAVI (Trans catheter aortic valve implantation). We have an exciting opportunity available for a nurse to come and join the TAVI team as a Band 7 TAVI Nurse Practitioner.
We are ideally looking for candidates who already hold their Physical Assessment Course and Non-Medical Prescribing as you will be required to assess, clerk and prescribe medications for elective and emergency patients.
The successful candidate will also be responsible for reviewing and assessing the patient prior to discharge and will need to be able to prescribe medications the patient will need to take at home. You will be expected to perform across the four pillars of advanced practice. This is a Monday to Friday Service which has been established for ten years. The department has recently celebrated performing 2500 TAVI procedures over this period of time.
This role is a pivotal part of the multi-disciplinary structural heart program at the Trust. The program caters for aortic intervention, septal closures leak closures and may expand into other areas. The post holder is expected to contribute to the provision of an efficient service and maintain high standards of care for patients with structural heart disease. Majority of the referrals received will be for patients requiring assessment to decide their suitability or Transcatheter aortic valve implantation (TAVI).
Main duties of the job
1. Provide specialist nursing advice Trust wide to support and facilitate high quality, individualised care to patients /clients and relatives/carers in collaboration with the multi professional team, in line with the local and national targets.
2. At all times ensure that the needs of the patient are placed at the centre of care delivery.
3. Facilitating a smooth patient pathway through management of the caseload of patient referrals and arranging various clinical investigations as required.
4. Coordinating the weekly MDT and scheduling of the procedures.
5. Attending weekly TAVI clinics and playing an active role in patient assessment, with the ability to interpret results, prioritise workload and take appropriate action when required. Coordinating the outcomes from consultant clinics.
6. Communicating with the wider MDT such as anaesthetics teams, bed managers, surgeons, valve companies and theatre teams as required, to arrange procedures.
7. Attending the TAVI procedures and undertaking practical training on heart valve loading in the catheter laboratory. - if desired, this can be facilitated.
