Belmont Recruitment are currently seeking a Care Assessor to work with a Lancashire local authority on an initial three month temporary assignment. This is a hybrid role working 37 hours per week, Monday to Friday.
Overview:
The Care Assessor will work within adult’s social care, carrying out needs led assessments and reviews in line with relevant legislation, policies and procedures. The post holder will work closely with colleagues, providers and partner agencies to ensure individuals receive appropriate, outcome focused support.
Main Duties:
* Undertake needs led assessments and reviews and construct, monitor and review outcome based packages of care in line with departmental policies and procedures
* Work closely with social work colleagues, internal teams, providers, carers and external agencies to deliver seamless adult social care services
* Ensure practice is compliant with the Care Act 2014
* Identify and escalate safeguarding concerns in line with the Safeguarding Adults Policy
* Support and empower individuals to make informed choices about their care and support
* Undertake capacity assessments and arrange advocacy support where appropriate in line with the Care Act 2014 and Mental Capacity Act 2005
* Maintain up to date knowledge of current social care policy and interventions
* Maintain accurate records and record decisions in line with statutory and departmental requirements
* Comply with relevant administrative and financial procedures
* Advocate on behalf of service users
Essential Criteria:
* Experience of undertaking assessments or reviews within adult social care
* Working knowledge of the Care Act 2014 and Mental Capacity Act 2005
* Understanding of safeguarding adults procedures
* Ability to work collaboratively with a range of professionals and agencies
* Strong communication, organisational and record keeping skills
