Gateshead Council is seeking a compassionate, skilled, and committed Learning Disability Social Worker to join our dedicated adult social care team. ? About the Role:
In this vital role, you will:
* Complete strengths-based assessments and create personalised care and support plans.
* Advocate for individuals with learning disabilities, ensuring their needs, rights, and aspirations are fully recognised and supported.
* Work closely with families, carers, health professionals, and community services to deliver holistic, integrated support.
* Regularly monitor and review care plans, making timely adjustments based on evolving needs and circumstances.
What We’re Looking For:
* Qualified Social Worker registered with Social Work England.
* Experience supporting adults with learning disabilities in statutory social care settings.
* Strong skills in assessment, planning, and collaborative working.
* Excellent communication, advocacy, and problem-solving skills.
* A proactive, person-centred approach and the ability to work independently or within a team.
