Who You Are An experienced Occupational Therapist with a passion for delivering high-quality, customer-centric services, and the ability to manage complex and challenging caseloads. You have at least two years of post-qualification experience, strong interpersonal and communication skills, and a commitment to continual professional development. You are registered with the Health and Care Professions Council and hold a valid UK driver's license. What the Job Involves As a pivotal member of the Reablement Team, you'll engage proactively with customers in their own homes to promote independence and minimize long-term care needs. You will conduct strengths-based assessments, collaborate with customers and their families to create and implement creative reablement plans, and deliver professional supervision and support to colleagues. Additional responsibilities include assessing and recommending minor and major adaptations, managing risk, maintaining accurate records, and supporting the safeguarding of adults according to legal and policy frameworks.
