Overview
An opportunity for an Occupational Therapist to work within the Home First service in North Staffordshire and Stoke-on-Trent. The service provides short‑term reablement interventions to support community patients, prevent unnecessary hospital admissions, and facilitate safe discharge from acute care.
Responsibilities
* Provide occupational therapy assessment and interventions for a wide range of disabilities.
* Work closely with multi‑disciplinary teams and external agencies to address care and support issues.
* Support patients in the home environment and assist with safe discharge from acute care.
* Evaluate rehabilitation outcomes and discharge or transfer individuals where appropriate.
* Identify and evaluate opportunities for innovation and improvement in service delivery.
* Maintain knowledge of NICE guidance, relevant NSFs, and the wider NHS agenda.
* Manage time and caseload effectively, supporting service users, families, and carers.
Qualifications
* Degree in Occupational Therapy or equivalent.
* Registration with HCPC/NMC.
* Fieldwork educators accreditation.
* Modules at Masters level.
* Evidence of further clinical study.
* APPLE accreditation.
* Membership of BAOT / COT.
Experience
* Post‑qualification courses in working with older people and people with physical disabilities.
* Attendance at relevant clinical networks or Special Interest Groups.
* Up to date field work/pre‑registration student training.
* Experience in using standardised assessments and outcome measures.
* Experience of negotiation and conflict management in clinical situations.
* Relevant experience to the role including assessment and intervention for a wide range of disabilities.
* Ability to manage time and caseload effectively.
* Supporting families and carers dealing with disability impacts.
* Evaluating outcomes of rehabilitation and supporting discharge/transfer decisions.
* Understanding of team dynamics.
Attributes
* Must be able to travel.

* Skilled Worker visas cannot currently be issued to Afghan nationals.
* Study visas cannot currently be issued to nationals of Afghanistan, Cameroon, Myanmar or Sudan.
