Main duties of the job

We are seeking to recruit a dynamic and innovative nurse with community experience to join our Children's Continuing Care Team for 9 months maternity leave cover. A secondment will be considered for the successful candidate.

As a Specialist Nurse, you will be an integral part of a team that sits within the wider Children’s community nursing service, working together to deliver this specialist service to children and families across the primary/secondary care interface. You will bring excellent communication, clinical skills and a desire to maintain and role model a culture of continuous improvement and innovation to inspire our workforce.

You will be responsible for developing and delivering specialist pathways of care as part of a multi professional team, working with care providers within Lambeth and Southwark and wider South-East London ICS to ensure children and families receive high quality care at home.

Detailed job description and main responsibilities

In partnership with members of the multidisciplinary nursing team the post holder will develop, implement and evaluate a seamless specialist service for children and young people requiring a continuing care package, ensuring that patients receive the highest standard of holistic care. They act as a lead specialist and will demonstrate a high level of expertise within the Children’s Community Nursing service, providing specialist advice, education and support to health care professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues. Also ensures the safety and well-being of services users, and always working in a manner that promotes dignity and human rights through the adoption of person-centred care principles.

Qualifications/ Education

Essential criteria

1. Post registration qualification in appropriate area of speciality or equivalent post qualification experience.
2. Leadership course
3. BSC/1st degree (Health related) or equivalent qualification/training and experience

Desirable criteria

4. To have or working towards a masters degree
5. Relevant community nursing qualification

Experience

Essential criteria

6. Proven evidence of management / leadership skills and autonomous practice
7. Experience of delivering patient advocacy
8. Demonstrate an understanding of: the National Framework for Children and Young People’s Continuing Care

Desirable criteria

9. Experience or participation in service development projects
10. Previous Community Experience

Skills/ Knowledge/ Ability

Essential criteria

11. Excellent verbal, written, interpersonal & communication skills
12. Time management and organisation skills

Desirable criteria

13. Ability to interpret and analyse complex data and findings

Professional/ Statutory Registration

Essential criteria

14. RN1 or RN Child Registered Nurse on the NMC register. To hold the appropriate level for the area of speciality.
