An exciting opportunity has arisen within Cwm Taf Morgannwg for a Band 6 Specialist Nurse to join our Cardiac Rehabilitation team at Prince Charles Hospital. We are seeking a passionate, forward thinking individual with a background in Cardiology.
Responsibilities
* Work as part of a team of specialist nurses, providing a high standard of nursing care to all patients accessing the Cardiac Rehabilitation service.
* Be a key member of the multidisciplinary cardiac rehabilitation team, providing a comprehensive cardiac rehabilitation service for patients following a cardiac event, through education and exercise programmes tailored to individual needs. This may take place in a variety of settings:
o Hospital, clinic or community based sessions
o Home visits
o Telephone and face to face support
* Empower patients to self-manage their condition by developing individual care plans and promoting opportunities for long-term self-management.
* Work as part of a multidisciplinary team, delivering a comprehensive cardiac rehabilitation programme for patients with cardiac conditions.
* Receive referrals from a variety of routes (e.g., wards, CCU, Cardiac Catheter Lab, tertiary centres, other specialist nurses and GPs) to determine suitability for entry to the service.
* Demonstrate excellent communication skills with patients and relatives, delivering information in a sensitive and empathetic manner.
* Participate in outpatient clinics, assess patients’ clinical condition and identify individual risk factors. Provide education and guidance according to each patient’s needs. Visit patients at home when required, ensuring Lone Worker policy is adhered to at all times.
* Produce specialist care programmes for each patient, liaising with GPs and Cardiologists as required. Be able to analyse findings from investigations and diagnostic testing (e.g., echocardiograms, ECGs, blood tests) and act in the patient’s best interest.
* Participate in patient education groups, providing specialist education, health promotion and advice to patients and relatives. Provide support during exercise programmes, working closely with the specialist physiotherapist.
* Work closely with the Team Leader and Consultant Cardiologists, discussing individual cases as required and seeking advice when necessary.
