Job title: Healthcare Plan Case Manager Location: Tewkesbury Salary: £25-27K Status: Full-time, permanent Purpose of Role: An exciting new opportunity has arisen for a Healthcare Plan Case Manager to join our team, supporting individuals through tailored, Trust Funded Healthcare Schemes. In this role, you will assess client needs, coordinate personalised interventions, and provide expert guidance to help clients achieve their health and wellbeing goals. Drawing on your professional background and strong communication skills, you will ensure each client receives the right support at the right time for sustainable recovery and long-term health.
