Senior Practitioner – Out of Hours
We are looking for a Senior Practitioner to work within Stamford Out of Hours delivering face to face consultations and telephone triage.
We are looking to recruit dynamic individuals with minor illness skills that will also compliment the existing team of GP's, ACPS and HCSW's.
This is a part time post 18 hours a week. We are looking to employ dynamic individuals with recognised qualifications and experience in minor illness and or experience working in GP practice, which will also complement our existing team of GPs, Advance Care Practitioner and Health Care Support Worker's.
Responsibilities
* Deliver a service 19:00pm-22:00pm Monday-Friday, weekends and bank holidays 08:00am-22:00pm to the community.
* See, treat, manage, discharge and/or refer patients of all age ranges to other LCHS services or partner services across Lincolnshire.
* Support other urgent care services such as UTC's, home visiting and the Clinical Assessment Service (CAS) which run 24/7 services.
* Daily GP support available remotely or in person as well as well-established referral pathways into specialities.
Qualifications
* Hold a non-medical prescribing qualification or willingness to work towards.
* Demonstrate experience in the treatment of minor illness.
* Meet the essential criteria in the person specification.
* Excellent leadership and communications skills.
