Based at Swindon Health Centre, you will have the flexibility to work within a team supporting both on-site and hybrid arrangements, depending on the role. HCRG Care Group is committed to fostering an agile workforce to meet organizational and service user needs.
The post holder will deliver high-quality care to a defined patient group within hospital or community settings under the guidance of a registered practitioner. Responsibilities include assessment, planning, organization, delivery, and evaluation of evidence-based, individualized patient care. The role involves undertaking delegated tasks and reporting to a registered practitioner.
This is a full-time position, 37.5 hours, Monday to Friday, with no weekend or bank holiday shifts.

Essential Qualifications and Skills:
* Foundation Degree or Higher Apprenticeship in Health & Social Care
* Level 2 functional skills or equivalent
* Evidence of a personal portfolio
* Experience in a care/health role and working within a multidisciplinary team
* Knowledge of Outpatients department and healthcare developments
* Organizational, customer service, clinical skills, and time management skills
* Ability to work independently and use hospital IT systems
* Solutions-focused approach and ability to build relationships with internal and external agencies
