Adults Intake & Assessment - Social Worker Pay rate between £30 and £34 (Umbrella) Vale of Glamorgan Council The Intake and Assessment team is a dynamic and exciting team with a varied and interesting workload. Our function is to: Provide a timely response to Safeguarding concerns, undertake sec 126 enquiries and liaise with partners to develop safe care & support plans Respond to people experiencing crises through assessment, support and family liaison and the commissioning emergency placements/care packages when necessary. Complete outcome focused assessments ensuring the full participation of service users and/or their carers Respond to urgent requests for equipment/manual handling concerns Promoting people’s wellbeing through the provision of Information, Advice and Assistance via our Wellbeing Matters Service. Review or assess and people who are known to Adult Services but whose needs have changed and may require a new assessment Develop Care & Support Plans in partnership with citizens to promote their wellbeing and social role within their community The job description refers to the principal duties and responsibilities of the Post. It does not necessarily list in detail all the tasks required to carry out these duties and responsibilities. • To undertake assessment of citizens in accordance with statutory duties under relevant legislation. • To plan and commission services to enable adults with care & support needs and carers with support needs to overcome the barriers to achieving the personal outcomes and monitor and review the care plans through co-production with citizens and carers • To work in partnership with citizens with care and support needs, carers with support needs and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them. • To negotiate and co-ordinate applications for resources and to work in partnership with internal and external partners both statutory and voluntary on behalf of citizens and carers • To undertake case management responsibility for a specific caseload and prioritise workload demands appropriately. • To maintain up to date electronic - case files and relevant documents, providing accurate records relating to performance management and other statistical data as required. • To prepare for and participate in supervision and Personal Development Review (Its About me) sessions with supervisor/ line manager, and identify personal development and training needs to maintain Registration with Social Care Wales. • To undertake training to support continuing professional development to ensure that the Council is able to retain the provision of its statutory functions, including a willingness for staff to undertake further training to become Approved Mental Health Practitioners, or other more specific, senior roles. • To supervise or mentor other staff or students where appropriate and offer input into training programmes and Team Practice Issues meetings where appropriate. • To take part in office duty rotas and other activities associated with a multi-disciplinary team. • Ensure compliance with the Council’s Financial Regulations, Standing Orders, Policies and Procedures • To implement the principles of the Council’s Equal Opportunity Policy whilst carrying out the above duties Benefits of working for Hoop: * 20 years’ experience of the Social Work market * Your local agency based in Cardiff * Welsh-speaking consultant available should you wish to converse in welsh throughout your job search * PAYE and Umbrella payroll options available – you choose! * Dedicated point of contact (support team to cover whilst on leave) * Twice weekly payroll * Actively supplying every Local Authority in South/West Wales and South West England * Provides free on-site quarterly training sessions (Contributes CPD) * Refer a friend scheme available * Loyalty Bonus to QSW's * Wellbeing sessions To apply for this role, you will need to hold - * A relevant Social Work Qualification * Registration with Social Care Wales * Enhanced DBS registered on the update service or be happy to apply for a DBS * 3 Years of referencing * Driving license with access to a car Given the nature of the role, only applications from individuals with a social work qualification and registration with either professional body - Social Care Wales / Social Work England can be considered.
