Social Worker – Adult Integrated Multi-Disciplinary Hospital Team
Sanctuary Personnel has a new permanent position available for a Social Worker within the Adult Integrated Multi-Disciplinary Hospital Team to work Full Time based in Hull.
The salary for this Social Worker job is up to £37,938 per annum.
* Promote and safeguard the welfare of adults at risk, families and their carers, within the ‘making safeguarding personal’ principles.
* Undertake proportionate assessments, determine eligibility, and assess risk. Plan care and support and make the best use of available resources to enable people to live the life they choose.
* Actively engage in team meetings/practice forums and other relevant meetings ensuring focused and purposeful social work is undertaken with all cases.
* Work with colleagues in the team to ensure the form of communications are clear and aligned with people’s needs. Demonstrate effective use of evidence-based social work practice.
* Work with colleagues in the team to ensure that case recording is up to date and that required information is available for a variety of purposes.
* Engage in the professional decision-making process with colleagues in relation to complex/high risk cases.
* Provide co-working support and advice to other staff members within the team.
* Engage in and advise on the professional decision-making process with adults at risk, families and their carers, and colleagues in relation to complex/high risk cases.
* Actively support and engage in practice development groups and the provision of information/professional opinion as required.
* Take a leadership role in managing the daily activity of the team.
Requirements of this Social Worker job:
* A social Work Qualification or equivalent.
* Understanding of relevant legislation.
* Registered or eligible for registration.
