Job summary: This is an excellent and rare opportunity for highly motivated and enthusiastic nurses to join the specialist palliative care team at East Lancashire Hospitals Trust. You will support the delivery of a specialist palliative care nursing seven-day service across hospital and community settings. Generalist palliative care experience is needed and must be demonstrated in your application form. You may be required to work across both hospital and community settings, subject to development and service demand. Staff are expected to work flexibly over seven days, including weekends and bank holidays.

Responsibilities
The post holder will contribute to the provision of a coordinated and comprehensive specialist palliative care service for patients, families and carers across ELHT. Responsibilities include:
* Maintain a caseload of patients with specialist palliative care nursing needs.
* Assess patients, formulate management plans and implement care by providing specialist nursing advice on complex pain and symptom management, and maintain associated records.
* Act as a specialist resource for advice on complex pain and symptom management to health care professionals across ELHT.
* Provide evidence-based specialist palliative care, advice and support to patients with cancer or other life-limiting illnesses, their families and carers.
* Manage workload to ensure delivery of care to meet patient needs.
* Ensure appropriate referrals to key health care workers when specialist palliative care intervention is complete.
* Work collaboratively with multi-professional teams across the health economy.
* Maintain accurate electronic patient records and communicate updates to the MDT.
* Collect and collate statistics as required in line with professional, national and organisational requirements.
* Act as a core member of the palliative care MDT to ensure assessment and discussion of new and complex patients.
* Participate in non-clinical aspects of the clinical nurse specialist role including audit, education and research.
* Provide specialist palliative care education and training to all members of the multi-professional team across care settings as required.
* Facilitate open and effective communication within the multi-professional team.
* Maintain good relationships with all disciplines within the trust and external partners.
* Use resources efficiently and report any concerns to the appropriate manager.
* Establish, utilise and maintain advanced communication skills in challenging situations with patients, carers, families and the multi-professional team.
* Be empathetic and reassuring when communicating highly sensitive information and advice to patients, families, carers and colleagues.
* Work within the competencies of V300 nurse independent prescriber if qualified.
* Contribute to clinical protocols and developments for the specialist palliative care service.
* Support line management and departmental improvements in the workplace.
* Ensure compliance with organisational policies and professional standards.
* Be an active member of groups representing needs of patients and staff.
* Contribute to the development and delivery of a seven-day service, working flexibly over seven days to meet service needs.
* Maintain up-to-date service information and compile statistical evidence as required.
* Contribute to the management of complaints and incidents.
* Work within the palliative care clinical governance framework.
* Contribute to achieving ELHT's objectives and targets for Specialist Palliative and End of Life Care.

Person Specification
Essential
* Registered adult nurse with the NMC register
* Demonstrable post-registration experience in palliative care
* On a relevant first degree pathway
* Demonstrable teaching qualification or equivalent experience
* Communication skills training / sage and Thyme
* Flexible to work 7 days and access to a car for work purposes
* Evidence of recent study or personal development
Desirable
* Advanced communication skills training
* V300 prescribing
* Understanding of palliative and end-of-life care local and national drivers
