We are looking for an enthusiastic and patient focused Health Care Assistant to join our friendly multidisciplinary team 3 day's a week. Exact days to be agreed.

Main duties of the job
* Undertake new patient health checks
* Support the practice nurse with health promotion programmes
* Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately
* Facilitate routine and 24-hour BP monitoring, advising patients accordingly
* Undertake wound care, dressings and other clinical tasks as required
* Support the practice nurse with the management of chronic disease clinics
* Carry out BMI checks as directed
* Act as a chaperone as required
* When trained, undertake venepuncture
* When trained, administer vaccinations under PSD
* Carry out ECGs as requested
* Ensure specimens are recorded and ready for onward transportation
* Provide support during minor operations as required
* Ensure all clinical rooms are adequately stocked and prepared for each session
* Ensure fridges are cleaned routinely in accordance with extant guidance
* Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy
* Deliver opportunistic health promotion where appropriate

We have a fantastic opportunity for a qualified Health Care Assistant to join our friendly group. We have 11 Partners, 17 Salaried GPs, 22 Nurses, 4 ANPs, 3 Paramedics, 2 social prescribers and First Contact Physios working across the group.
Job responsibilities
* To develop, implement and embed health promotion and well-being programmes
* To implement and evaluate individual treatment plans for chronic disease patients
* To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health
* To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
* To provide wound care (ulcer/Doppler etc.) to patients
* To request pathology services as necessary
* To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
* To maintain accurate clinical records in conjunction with extant legislation
* To ensure read codes are used effectively
* To maintain chronic disease registers
* To chaperone patients where necessary
* To assist GPs with minor surgery when required
* To prioritise health issues and intervene appropriately
* To support the team in dealing with clinical emergencies
* To recognise, assess and refer patients presenting with mental health needs
* To implement vaccination programmes for adults and children
* To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
* To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
* To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
* To support the clinical team with all safeguarding matters in accordance with local and national policies
* To deliver opportunistic health promotion where appropriate
* To support junior members of the nursing team, providing guidance when necessary
* To participate in local initiatives to enhance service delivery and patient care
* To support and participate in shared learning within the practice
* To continually review clinical practices, responding to national policies and initiatives where appropriate
Person Specification
Experience
* Experience of working within a nursing team
* Experience working with patients
* Experience as a HCA in a health care setting
* Experience carrying out venepuncture, ECGs, simple dressings and physical health monitoring
* Experience working in primary care or general practice, with HCA training or competence. Previous experience using clinical systems (e.g. EMIS/SystmOne), knowledge of QOF, and evidence of ongoing continuing professional development would be advantageous.
Qualifications
* Healthcare qualification/experience (level 3 or 4) or working towards gaining equivalent level
* Experience of working in a healthcare setting
* Experience of working in a primary care environment
* Wound Care / Removal of sutures & staples
* ECGs
* Venepuncture
* New patient medicals inc. Height, weight, BP, Pulse
