We are seeking a motivated and flexible administrator to support our team in daily operations. Responsibilities include answering and directing phone calls, taking referral details, advising on response times, supporting data collection and analysis, stock management, and assisting clinicians to deliver high-quality care.

About the role

The Pathfinder Team is a specialist community team within North Devon District Hospital, comprising professionals from therapy, nursing, and paramedic backgrounds. Our remit includes holistic patient assessments, procurement of care, advice, equipment issuance, and therapy support for groups such as:

* Short stay admissions ward discharges
* Frailty Acute Hospital at Home
* End of life care discharges

Our work is time-sensitive and crucial for patient flow, offering a rewarding and varied experience. We collaborate closely with community partners to ensure seamless care transitions from hospital to home or other destinations.

We seek a forward-thinking, dynamic, motivated, and creative individual who can work well in a team and under pressure. Experience with trust computer systems and standard applications is beneficial; training will be provided. We are a friendly, flexible, and supportive team committed to mutual support through challenges.
