Job overview

An exciting opportunity has arisen for an enthusiastic, flexible, hardworking and motivated individual to join our Co-ordinator team within York Community Response Team.

Within your role, you will form a crucial part of the Multi-Disciplinary Team, delivering a high standard of health care to patents living within their own homes, facilitating short term rehabilitation, reablement and recovery in order to avoid unnecessary hospital admissions and enable patients to recover following an acute hospital stay.

Within this role, you will be first contact for patients, families and health & social care partners across the local system. You will be responsible for supporting the patient pathways including co-ordination of assessments / daily visits and supporting the day to day running of the service.

Main duties of the job

The post holder will proactively support all CRT referrals, ensuring each stage of their pathway is fully coordinated, planned and booked. Will act as a main point of access to the service providing contact for patients, carers and wider healthcare team to provide (in conjunction with the clinical team), high quality patient care through proactive identification of needs which require a level of clinical knowledge, triage, allocation and discharge planning.

Collaborate and communicate with the wider health and social care team, acting as a resource to ensure the provision of appropriate services to maximise the health and wellbeing of the patient, improving the quality and efficiency of healthcare delivery. The post holder will be responsible for the coordination of the administrative function within the complex CRT pathways arranging assessments, reviews, discharge planning, transfers of care and multi-disciplinary team discussion in a timely manner, to facilitate unnecessary hospital admission and timely hospital discharge. Will be expected to manage and prioritise the allocation of support worker visits and work flexibly to meet the needs of the service, which can often be demanding and unpredictable.

Education, Qualifications and Training

Essential criteria

1. Educated to GCSE / O’level standard in core subjects or evidence of working at this level

Desirable criteria

2. Knowledge of data bases and spread sheets

Experience and Knowledge Required

Essential criteria

3. Minimum 2 years’ experience within the NHS or similar environment (e.g. social services or other public services)
4. Evidence of ability to develop effective working relationships with different levels and disciplines, internal and external to the organisation
5. Insight into rehabilitation and re-ablement issues

Desirable criteria

6. Experience of audit and data collection
7. Administrative support for clinical team including use of Microsoft Office and experience of patient information systems e.g. CPD, SystmOne
8. Insight into roles and responsibilities

Skills and Attributes

Essential criteria

9. Excellent organisational and prioritising skills
10. Reliable, adaptable to change and quick to learn and have the ability to work on own initiative or as part of a team.
11. Good verbal and written communication skills including excellent telephone manner
