We have an exciting opportunity for an experienced Social Worker to join our Community Team for People with Learning Disability!
This is a full‑time, permanent role with hybrid working, and a salary of £40,192 to £44,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

Responsibilities
* Located within a multi‑disciplinary community team you would be supporting young people with learning disabilities and their families and carers to make a positive transfer to adult services. This would include liaising with Children's services and any other involved agencies.
* You would be involved in carrying out assessments, undertaking care reviews and supporting care planning.
* You would take part in a duty rota to deal with incidents, safeguarding alerts and information requests.
* You would work as a Social Worker to protect and promote the interests of vulnerable people.
* Undertake safeguarding investigations, Deprivation of Liberty Safeguards assessments, capacity assessments and best‑interest decisions for adults with a learning disability.
What we are looking for
* A qualified Social Worker with a qualification such as a BA, BSC Diploma or equivalent.
* Current registration with Social Work England.
* Experience of working in a health and social care environment.
* Good understanding of safeguarding protocols.
* Knowledge of relevant legislation.
* Committed to working co‑operatively as part of a team.
