About the Service
The Hampshire, Frimley, and Isle of Wight Perinatal Mental Health Service is a well-established and pioneering specialist service, comprising an expert multidisciplinary community team and a mother and baby inpatient unit. The service is an early implementer site for Maternity Mental Health Services, broadening the scope of specialist interventions available for women and birthing people, and ensuring an enhanced care pathway.
The Perinatal Community Team provides a specialist community service for the assessment and treatment of women with severe mental health problems during pregnancy and up to two years postnatal.
Role Overview
We are seeking an experienced Mental Health Nurse, Occupational Therapist, or Social Worker with an interest in Perinatal Mental Health. The role is based within our South East Hampshire locality team at Fareham Community Hospital. A car driver is essential.
The Perinatal Community Mental Health Nurse/Practitioner will undertake comprehensive assessments, manage a complex caseload, provide practical advice and support, including risk management, psychological interventions, and safeguarding. Care coordination and planning within the multidisciplinary team are crucial to deliver patient-led, collaborative, high-quality care.
The successful candidate will work closely with locality colleagues and the wider perinatal team, including parent-infant practitioners, psychologists, and medical staff, to ensure the best care and experience for service users. They will also collaborate with external organizations to ensure robust joint working where needed.
