We are seeking compassionate and skilled Primary Care Nurses to provide primary care services in a supportive, whānau-centred environment. You will work closely with our integrated care team to deliver exceptional care through assessments, minor surgery support, health screening, and patient follow-ups.

Role Overview: As the Registered Nurse you will facilitate high-quality care coordination for individuals and whānau with complex health needs. Your work will involve collaborating across health and social services to ensure seamless transitions of care, ultimately improving health equity.
Key Responsibilities:
* Patient assessments
* Provide health education, lifestyle counselling, and chronic disease management
* Coordinate patient care with GPs, community services, and other health providers
* Support immunisation, family planning, and sexual health services
* Advocate for patients' needs with external agencies
* Engage and support a ‘circle-of-care’ approach to holistic wellbeing.
* Current Registered Nurse with an unrestricted annual practicing certificate.
* Primary Care Nursing experience is ideal with experience in Immunisations, Sexual Health, Diabetes, Skin and Cardiac.
* In-depth understanding of health equity in Aotearoa and commitment to Te Tiriti o Waitangi.
* Exceptional communication, organisational and critical appraisal skills.
* Ability to independently manage complex workloads and demonstrate innovation in care delivery.
* Strong ability to build trust with whānau, promoting self-determination and autonomy.
The Role
We are looking for Primary Care Registered Nurses who are able to work up to 0.8 FTE [32 hours per week].
Our clinic is open 8.30am to 5.00pm Monday to Friday with one late per week working 9.30am to 6.00pm.
Must live in Hawkes Bay and have experience working in our local community.
