Gender Plus is a specialist service providing gender-affirming care to people across the
lifespan. We work in a holistic, collaborative, and person-centred way, holding the wellbeing
and autonomy of those we support at the heart of everything we do. Our Gender Plus
Hormone Clinic is rated Outstanding by the Care Quality Commission (CQC) — a reflection
of the dedication, expertise, and compassion of our entire team.
The Gender Plus Hormone Clinic provides specialist hormone-related clinical reviews and
psychosocial clinical appointments for people exploring or undergoing gender-affirming
hormone therapy. The Hormone Clinic operates from multiple bases across our network, and
this post can be linked to any of the UK sites.

3. Job Purpose
The Qualified Mental Health Nurse (RMN) will work as an integral member of the Gender
Plus Hormone Clinic multi-disciplinary team (MDT). The post-holder will act as a designated
clinician for hormone clinic review appointments, carrying out psychosocial and clinical
assessments, risk assessments, and care coordination for individuals attending the
Hormone Clinic. They will work under the management oversight of the Lead Nurse
Specialist and in close collaboration with the Clinical Psychologist and wider MDT.
This post is ideal for a qualified and registered mental health nurse with a commitment to
gender-affirming care, who wishes to develop specialist clinical expertise in a progressive
and compassionate transgender healthcare setting. We also welcome applications from
newly qualified mental health nurses or those who will qualify shortly.
4. Key Responsibilities and Duties
4.1 Clinical Work
• Act as a designated clinician for hormone clinic review appointments, working
alongside the Lead Psychosocial Clinical Nurse Specialist, and/or other senior
clinicians.
• Conduct structured risk assessments and develop appropriate, evidence-based
risk management plans, including assessment of mental health risk, safeguarding
concerns, and risk related to the person’s gender care.
. To complete basic baseline nursing procedures such as taking blood pressure,
taking height and weight measurements and administering intramuscular
injections.
• Communicate care plans clearly and compassionately to service users, ensuring
they feel heard, respected, and involved in their care.
• Coordinate care and liaise with other professionals involved in a service user’s
care (e.g. GPs, CAMHS, mental health teams, social care, endocrinology).
• Prepare and complete clinical letters and reports to a high professional standard.
• Maintain accurate, timely, and professionally appropriate clinical records in
accordance with NMC standards, service policies, and information governance
requirements.
• Contribute to clinical discussions and MDT meetings, sharing assessments and
clinical observations in a structured and professional manner.
• Work in accordance with Gender Plus policies for confidentiality, consent,
safeguarding, and information governance.
4.2 Risk Assessment and Safeguarding
• Identify and respond appropriately to risk, safeguarding concerns, and acute
mental health needs arising during clinical appointments.
• Escalate concerns promptly and appropriately to the Lead Psychosocial Clinical
Nurse Specialist
• Liaise with external professionals and statutory services as required to ensure
service user safety.
• Complete relevant risk documentation in line with service protocols and NMC
professional standards.
• Participate in safeguarding training and demonstrate up-to-date knowledge of
safeguarding responsibilities for both adults and young people where applicable.
4.3 Care Coordination and Pathway Support
• Support service users through their gender-affirming care pathway, providing
continuity, coordination, and clinical advocacy.
• Work closely with endocrinology colleagues, the Lead Nurse Specialist, to ensure
integrated, seamless care.
• Contribute to the development and review of care plans, ensuring these are
person-centred, evidence-based, and reflective of the service user’s goals and
circumstances.
• Provide psychoeducation and information to service users and where
appropriate, their families or support networks.
4.4 Research, Audit, and Service Development
• Assist with data collection, audit, and service evaluation activities as directed.
• Support the team in reviewing and improving clinical pathways and assessment
processes.
• Maintain current knowledge of evidence, guidance, and good practice relating to
transgender health care, mental health, and neurodiversity.
• Contribute to the development of resources and information materials for service
users.
4.5 Professional Development and Supervision
• Maintain current registration with the Nursing and Midwifery Council (NMC) and
adhere to the NMC Code of Conduct at all times.
• Engage actively in regular clinical and management supervision.
• Participate in team meetings, reflective practice sessions, and CPD activities.
• Demonstrate a commitment to ongoing self-development and to the ethos and
values of Gender Plus.

5. General Duties
• Maintain NMC registration and comply with revalidation requirements.
• Comply with all service policies, procedures, and guidelines.

• Attend statutory and mandatory training as required.
• Contribute to the maintenance of a safe working environment for service users
and colleagues.
• Undertake any other duties commensurate with the grade and scope of the post,
as directed by the line manager.
