Responsibilities
* Manage a caseload of individuals with mental illness and social care needs, completing assessments under the Care Act 2014.
* Conduct holistic, strength‑based assessments and commission personalised support plans.
* Act as Care Coordinator, coordinating care across agencies, creating care plans and undertaking risk assessments.
* Provide a least restrictive and preventative approach to keep service users in their homes, avoiding hospital admissions.
* Work within a multi‑disciplinary team, ensuring risk is managed and safeguarding is upheld.
* Participate in a blended approach to office and home working, using IT as required.
Qualifications
Qualified social worker with relevant experience in mental health and social care. Registration with the Health & Care Professions Council (HCPC) or equivalent.
