Job Summary
To work within a newly formed Hospital at Home Team for patients aged 65 plus to prevent admission into hospital. To be an integral part of the MDT providing direct psychological assessment, formulation and brief intervention with patients. To provide the HAHT with reflective practice, support with risk assessment and training and development opportunities in psychological ways of working with the patient group and carers.
Responsibilities
* Provide highly specialist psychological assessment of patients with highly complex conditions based on the appropriate use, interpretation and integration of data from a variety of sources, including neuropsychological testing.
* Undertake risk assessment and risk management for individual patients and provide specialist advice to other professionals on psychological aspects of risk assessment and risk management.
* Formulate plans for highly specialist psychological therapy and/or management of patients’ mental health conditions based upon an appropriate conceptual framework and evidence-based practice. Negotiate the implementation of such plans with patients and/or their carers.
* Accountable for own professional practice in the delivery of highly specialist psychological care to older people under the care of the Hospital at Home Team (HaHT) with highly complex mental health or dementia which may include challenging behaviours.
* Exercise supervisory/professional responsibility for other psychological therapists.
* Provide leadership in multi‑disciplinary training and development, service audit and development, service evaluation and research, and ensure that a psychological perspective is brought to service redesign and cultural change.
Qualifications
* HCPC Registered Practitioner Psychologist.
* Experience of working with older adults.
* Doctorate level training in applied psychology (or its equivalent) as accredited by the BPS.
