Responsibilities
* Provide support and advice on discharge in collaboration with the Integrated Discharge Team and ward teams, including completing referrals to other services, working to tight deadlines, and communicating with patients, families and the wider MDT.
* Act as the main point of contact with ward teams, patients and families, helping to reduce length of stay.
* Coordinate the ward’s discharge processes and ensure patients, families and carers are involved from admission to discharge.
* Communicate regularly with patients and relatives to promote understanding of discharge planning and progress, serving as the focal point for discharge information and planning for families, patients and the MDT.
* Liaise with internal and external agencies to provide a holistic approach to discharge.
* Ensure processes are followed to enable timely access to social care, rehab facilities and community services; identify potential problems early and develop initial plans to address them, escalating to relevant teams as necessary.
