Job Overview
Applications are invited for a Patient Pathway Coordinator within the Diabetes and Metabolism Department, specifically supporting the Obesity Service at Barts Health NHS Trust, working across The Royal London Hospital, Barts and Mile End Hospital. This role is a one‑year fixed‑term post commencing 1 April 2026.
Responsibilities
* Coordinate patient pathways from referral through treatment, discharge, and follow‑up in outpatient services.
* Act as a consistent point of contact for patients, families, clinicians and wider teams, providing clear information about appointments and care journeys.
* Manage, adjust and reschedule clinic appointments, ensuring clinic slots are used effectively in line with agreed clinic patterns.
* Work closely with clinicians and clinical team leaders to amend, cancel or reschedule clinics, adjust capacity, and meet local and national targets.
* Maintain accurate and up‑to‑date patient records using NHS systems, monitoring waiting lists and cancellations.
* Identify delays early, escalating risks of breaches to ensure compliance with national standards.
* Act as the main administrative contact for patients and clinicians across the pathway.
* Provide cross‑cover across the Trust’s multiple sites as required.
