Job Overview
An exciting opportunity has arisen due to the expansion of the team for a Band 4 Diana Children’s Specialist practitioner, Special Needs Education. The position is based at Ash Field Academy special school, working Monday to Friday, Term Time only (08:30–15:30). The successful candidate will be part of the Diana Children’s Community Service, providing education and healthcare support to children and young people with complex healthcare needs, including life‑limiting and life‑threatening conditions.
Responsibilities
* Provide education and healthcare support in accordance with individual care plans, EHCP, IEP, and statements.
* Work as part of a nurse‑led service caring for children/young people with life‑limiting illnesses and/or technological support under the supervision of registered nurses.
* Support qualified nursing staff and the SENCO in reviewing each child’s care plan.
* Collaborate closely with parents/carers to deliver care in line with the individual care plan.
* Contribute to the assessment of children’s and young people’s health and wellbeing alongside qualified staff.
* Monitor and evaluate health and care management as well as educational targets, ensuring EHCP adherence.
* Take lead role alongside registered nurse colleagues caring for more complex children.
* Provide day‑to‑day health advice to parents/carers and respond to incidents.
* Maintain professional records ensuring confidentiality.
* Supervise and provide induction for new team members, supporting preceptorship programmes.
* Contribute to systems and processes for safe care delivery, duty rota coordination.
* Report changes in condition and issues to the registered nurse promptly.
* Communicate effectively with parents/carers regarding health and care management.
* Contribute to clinical governance and participate in annual clinical audit programme.
* Follow the Leicestershire Medicine Code for handling and storage of medicines.
* Contribute to child protection risk management in line with LPT and school procedures.
* Meet competence requirements, attend training, and engage in professional development.
* Undertake any other reasonable duties, including staff meetings, parent evenings, and training.
Qualifications
Knowledge
* Understanding of child development or education to level 3 or equivalent.
* Understanding of equal opportunities.
Experience
* Experience of working with children and/or disability.
* Experience of child development.
Skills
* Effective communication skills both written and verbal, including the ability to receive and provide complex/sensitive information.
* Desire to develop clinical skills and implement developments within the service.
