A fantastic opportunity to join our District nursing Hub in the Community to work as part of the District Nursing Team. This is an exciting time for DN Hub supporting the District nursing services and unplanned services within the community through telephone triage and support.
We are now looking to appoint a staff Nurse into our friendly multi‑disciplinary team who work closely with other community services to provide wrap‑around care for patients registered with a North Lincolnshire GP.
Main duties of the job
The hours available include a mixture of shift patterns that will be rotational to cover the Hub from 0715 hrs till 2115 hrs across seven days a week.
This post will suit staff nurses who enjoy working within a developing service and are motivated and enthusiastic. This post is based on telephone triage and the post holders are not expected to deliver face‑to‑face care. Therefore, these roles will provide working arrangements that may suit an individual whose personal circumstances or career needs do not facilitate them providing face‑to‑face care. We are keen to receive applications from people who come from a Black, Asian or other minority ethnic background or identify as having a disability. The post holders will be expected to complete appropriate triage training as needed by the service.
The purpose of the service is to signpost and assist the patients under district nursing service and unplanned service through clinical triage, advice, guidance and support that meets their needs.

Person Specification
Education & Qualifications
* Level 1 Registered Nurse
* Diploma or degree in nursing
* Qualified mentor (ENB 998 or mentorship award or equivalent) or working towards
* Evidence of post‑registration development in the relevant specialty
Occupational Experience & Abilities
* Sound and broad range of clinical skills/knowledge within the specialty
* Minimum 1 year post registration experience
* Face‑to‑face/tele‑triage experience
* Previous community nursing experience

Salary: £32,073 to £39,043 a year pro‑rata
