Overview

Within the Social Work team at Keech we have an exciting opportunity for a qualified adult social worker to join us with post-qualification experience and a desire to develop specialist palliative care experience within a multi-agency hospice team. The role specialises in working with adults and their loved ones with a palliative illness, from diagnosis through end of life care. You will use specialist Social Work skills to support people in dealing with the impact of what is happening to them, including themes of loss and bereavement, and to promote and support their choice and control in life and in death. You will support patients admitted to the hospice in-patient unit and those in the community. You will bring your Social Work expertise to support the management of complex safeguarding, mental capacity and best interests within a multi-agency team to ensure access to individualised and holistic support for patients and their loved ones. Keech has a strong value base and we work as a team to develop services that meet the needs of patients and their families.


Responsibilities

* Provide specialist Social Work support to adults with palliative illness and their families, across in-patient and community settings.
* Contribute to complex safeguarding, mental capacity and best interests decision-making within a multi-agency team.
* Apply professional knowledge to promote person-centred care, choice and control at all stages of illness, including end of life.
* Support assessment, planning and coordination of holistic care needs in collaboration with colleagues and external services.
* Engage in Bereavement and loss support as part of the care pathway.


Qualifications

* Recognised Social Work qualification and registration with Social Work England.
* Experience or knowledge of palliative care or working with people with complex health conditions and care needs.
* Ability to work effectively in emotionally challenging situations.
* Excellent assessment and communication skills.
* Knowledge of relevant legislation and ability to apply it in the role.

We support adults in Bedfordshire and children and their families in Bedfordshire, Hertfordshire, and Milton Keynes. Care can be provided at home, in hospice, or wherever needed. Salary and Benefits

* Starting salary - £25,801 per annum (FTE £43,003) with room to develop and progress through the pay grade banding to £29,191 (FTE £48,653) on a competency based framework.
