About the Role
This role is based within the Exeter South & West cluster, supporting rehabilitation to patients, whether that be as part of a supported hospital discharge or proactive admission avoidance. With a supportive MDT, including Physiotherapists, Occupational Therapists, Nurses and Assistant Practitioners, the team strives to provide patient‑centred care to enable individuals to achieve their full potential.
The successful post holder should be highly motivated with a willingness to be adaptable to service needs and pressures as the post will support patients within both the UCR team and community rehabilitation team. They will ideally have experience in a health and social care setting.
The South & West cluster covers the areas of Broadclyst, Pinhoe, Topsham, Exminster, Alphington, St Thomas and Starcross. Access to a suitable vehicle is essential for this role.
Working Pattern: 34 hours per week. The team supports patients in the community between 8 am and 8 pm, 7 days a week. Staff work a range of shift patterns within these times. Depending on your shift pattern and working hours, you may also receive additional enhancements to your pay.
