We are looking for a motivated, compassionate and reliable Nursing Support Worker to join our dynamic Community Nursing Teams based in Pembroke Dock, District Nursing Team. You will work as an integral part of a multidisciplinary team delivering high-quality nursing care to people in their own homes, residential facilities and community settings.

This is a permanent position working over a 7-day period, including weekends and bank holidays, covering an 8am-6pm service.

This is a rewarding role where you can make a real difference to patients and their families by supporting independence, promoting wellbeing and helping people remain safely at home.

Due to the rural area public transport is limited. The ability to travel within geographical area to meet the needs of the service are required.

Main duties of the job

As a Band 3 Nursing Support Worker, you will:

* Provide high-quality, person-centred care under the direction of Registered Nurses.
* Assist with wound care, venepuncture, pressure area care, catheter care and other delegated clinical tasks aligned with your competencies.
* Support patients with activities of daily living, including personal care, nutrition, hydration and mobility.
* Accurately document care in line with NHS Wales and Hywel Dda UHB policies.
* Record observations and care interventions.
* Work collaboratively with colleagues to ensure safe and effective service delivery.
* Travel within the local community to visit patients.


About us

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in:

* Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
* Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
* Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
* 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
* Numerous mental health and learning disabilities services

Person Specification


Qualifications and Knowledge

* Level 3 qualification in a health care related subject Or Equivalent experience and can evidence work based competency demonstrating theoretical knowledge, and the ability to undertake clinical care duties relevant for the role which is gained through experience and worked based training/short courses.
* Knowledge of the Code of Conduct for Healthcare Support Workers in Wales and All Wales Delegation Guidelines (HEIW).
* Knowledge of relevant policies and procedures within a healthcare setting for example the principles of Safeguarding adults/children to keep patients within their care safe, and confidentiality for example General Data Protection Regulations (GDPR) .


Experience

* Experience of working within a relevant heath or social care environment, providing direct patient care
* Experience of working with patient groups relevant to area of practice.
* Experience of working within the NHS.

* Welsh Speaker (Level 1)
