Job Overview
Supported as part of the 0‑19 Healthy Child Programme in Norfolk, delivering high quality, patient‑centred care through collaboration, innovation and shared expertise.
Responsibilities
* Conducts health assessments and develops, implements and evaluates individual care plans for children and young people aged 0‑19 and their families.
* Works collaboratively with multidisciplinary teams, community agencies and services (Early Childhood, School Services, Health, Voluntary organisations).
* Provides face‑to‑face and virtual visits, including digital drop‑in clinics and video contacts.
* Parallels local and national targets, delivering public health programmes, health promotion and early intervention initiatives.
* Advocates and promotes inclusive, evidence‑based health and wellbeing, reducing health inequalities.
* Participates in safeguarding, child protection case conferences and protects vulnerable families.
* Supports service delivery at the Single Point of Access and, as required, at other localities.
* Provides mentorship to nursing and other discipline students and supports clinical supervision.
* Uses digital technology and platforms (JustOneNorfolk, Just One Number) to enhance self‑care and service delivery.
* Maintains clinical records, data protection, infection control and NMC code of conduct compliance.
* Adheres to budgeting, risk management and infection control policies.
* Travels independently across Norfolk, requiring a full driving licence and personal transport.
* Contributes to audits, research, quality improvement and service design projects.
Person Specification
Qualifications & Training
* RGN/RMN/RSCN/RN – LD or Registered Midwife
* Teacher/mentor qualification (e.g., Level 998 or equivalent)
* Management and leadership qualification
* V100 non‑medical prescribing or willingness to work towards it
* Evidence of completion of migratory portfolio to demonstrate dual qualification or willingness to work towards it
Experience
* Experience working with children and families
* Experience profiling localities and identifying needs
* Experience working in community settings
* Experience in a multidisciplinary setting
* Experience with safeguarding procedures
* Experience in mentoring
Knowledge & Skills
* High level of clinical skills and ability to evaluate health needs
* Commitment to developing excellent services for children’s improved outcomes
* Current knowledge of local and national health visiting policies
* Excellent communication with children, parents, early years and health organisations
* Proficiency with SystmOne and digital health platforms
* Leadership and line management skills
