In partnership with Eston Primary Care Network (PCN) and South Tees Hospitals, we are proud to offer an exciting cross-sector pharmacist opportunity working seamlessly across care settings. This role will act as a vital link between primary and secondary care, leading on initiatives that ensure safe, efficient, and highquality transitions for patients moving between services.
This is a challenging and dynamic role that will allow successful applicants to strengthen their clinical knowledge and expertise, whilst also enhancing their consultation and assessment skills. Additionally you will also be able to develop your negotiation, influencing, leadership and management skills. In doing so, you will be required to;
1. Act as a positive role model for medicines management and medicines optimisation for pharmacy colleagues and other health care professionals,
2. Utilise transcribing or prescribing skills effectively to enhance patient care and patient safety,
3. Continually monitor performance and productivity, developing and piloting new ways of working to improve efficiency and patient outcomes,
4. Be involved in the preparation of policies and procedures,
5. Be involved in risk reporting and management,
6. Provide relevant and up to date education and training to pharmacy staff and other healthcare professionals.
Main duties of the job
The pharmacy department at South Tees Hospitals NHS Foundation Trust will be dedicated to your development, supporting you in your new role with various internal and external educational opportunities available to help you further develop both your clinical and professional skills.
The department hosts over 200 members of staff across both sites and provides a stimulating, friendly, and professionally rewarding environment for staff to develop their clinical expertise and professionalism.
The successful candidate will be full time ( hours)- 27 hours for Eston Primary Care Network with the remaining hours worked at South Tees Hospital (compressed hours welcomed). Additionally the successful candidate will be included in extended hours, weekend and on-call rotas, which are currently;
7. 1 day weekend working once every 10 weeks (reimbursed with a weekday off the following week),
8. On call duties on a rolling rota basis, approximately 1 in 40.
South Tees NHS Foundation Trust is situated in the Tees Valley just a step away from the North Yorkshire Moors and coast and is well connected nationally and regionally by road, rail and air. Hospital services are provided from two main sites, the James Cook University Hospital in Middlesbrough and the Friarage Hospital in Northallerton. The James Cook University Hospital is designated as a Major Trauma Centre and as a Tertiary Care centre.

Essential criteria
9. Experience in clinical pharmacy within a hospital or primary care setting.
10. Experience developing strong professional relationships with other healthcare professionals
11. Experience of contributing to relevant research, audit and service improvement projects
12. Experience of involvement in MDT service development, or MDT clinical meetings
Desirable criteria
13. Experience in mentorship and training for other staff
Qualifications and Training
Essential criteria
14. Masters Degree in Pharmacy or equivalent
15. Registered Pharmacist with the General Pharmaceutical Council
16. Evidence of CPD and ongoing education
17. Independent Prescribing or working towards
Desirable criteria
18. Diploma in Clinical Pharmacy or equivalent relevant postgraduate clinical qualification
Knowledge and Skills
Essential criteria
19. Ability to apply theoretical knowledge to clinical practice
20. Involvement in development of evidence-based guidelines, policies and protocols
21. Strong knowledge of medicines information resources
Disclosure and Barring Checks (DBS)
