This is an exciting opportunity to join Angus Heath and Social Care Partnerships Adults with Physical Disabilities Service.
You will be part of a team of social workers working closely with other health professionals. The ability to promote and encourage a collaborative working approach is essential.
Your duties will include providing comprehensive person-centered assessments for people with physical disabilities, to promote their independence, involve them in their local community and achieve their full potential. You will also conduct carers assessments. You will complete and maintain appropriate records. You will manage a caseload, carrying continuous responsibility for the assessment and evaluation of the provision of care and will participate in the continuing development of the service.
At times our Social Workers will be required to support people within a legal framework; this includes working with the Adults with Incapacity (Scotland) Act 2000, Mental Health (Care and Treatment) (Scotland) Act 2003 and the Adult Support and Protection (Scotland) Act 2007. This statutory work allows Social Workers to work in more depth with supported people, build meaningful relationship and support them to make safer choices with positive results.
